Appointments: To schedule an appointment, click the link below and follow the prompts. This is a free service but you must be 18 years old to receive the vaccine we administer. You are welcome to choose any provider who is available. We don’t want any vaccines to go to waste so, if you cannot make your appointment time, kindly contact us 48 hours in advance via email: vaccinatesherwood@gmail.com. Please arrive 15 minutes early and do not bring anyone with you who does not have an appointment.

Vaccinate Sherwood
Paperwork: You will need to print and fill out the pre-screening and consent form and bring it with you to your appointment. It is found below or in the confirmation email you receive after scheduling your appointment. If you cannot print it, please arrive extra early to allow time to fill out the necessary paperwork.
Parking: There should be plenty of parking but if needed, additional parking is available in the adjacent vacant lot on Handley Street during these clinic times only.
Health: As you know, one must be in good health to receive the COVID-19 vaccination. If you are not feeling well, please contact us to reschedule. Do not schedule, if you have had severe allergic reactions to any other vaccines. It is also best to not take any Ibuprofen or non-steroidal anti-inflammatory medications for one week prior to getting your vaccine. We ask that you please wear a mask for the duration of the appointment.
